What Is Essential Tremor
Essential tremor is a movement disorder that causes parts of your body to shake. This kind of shaking isnt controllable, and you cant keep yourself from doing it. Essential tremor usually affects your hands and arms but can also affect your head, voice and other body parts.
In its advanced stages, this condition can severely disrupt some of the most basic tasks and parts of life, such as eating, drinking and dressing yourself. Many people also struggle with feelings of embarrassment or anxiety about the symptoms of this condition.
What Are The Symptoms Of Essential Tremor
The key symptom of essential tremor is shaking usually your hands when youre trying to use them. That shaking can take different forms and usually happens under certain circumstances. The forms and circumstances where tremors are likely include:
- Action tremor: This is a form of essential tremor that happens during actions, such as reaching for an object.
- Postural tremor: This is shaking that happens when you hold part of your body in a specific pose, such as holding a hand outstretched and keeping it at the same height.

When You Have Fluorouracil

It was Christmas morning in 2018, and Karen was in her kitchen trying to make huevos rancheros. The Mexican breakfast dish was a tradition she always made for the family, but this year it wasnt working. Her hands were shaking so badly that she couldnt fry the eggs. After ruining three or four eggs in a row, her daughter-in-law stepped in and asked if she could help. Karen was devastated.
It might seem like a minor thing to not be able to fry eggs, but for Karen it was the culmination of several years of a neurological disease called essential tremor that kept getting worse. Then Karen was diagnosed with breast cancer in 2018. She needed surgery, chemotherapy, and radiation. That treatment is hard for anyone, but for her, there was an added hurdle.

Parkinsons disease is a neurological disorder that causes significant problems with the movement or motor system because of a problem with a brain chemical called dopamine. Classic symptoms are tremor and shaking or trembling hands. Tremors worsen over time.
Unlike essential tremor, the tremor happens mostly when you are at rest. People with Parkinsons disease have stiff, slow movements, balance problems, and are at risk of falls. The tremor can also affect the legs. Other symptoms of Parkinsons include dementia, mood disorders like depression, psychosis, and hallucinations.
Treating Parkinson’s disease
Parkinsons is treated with a medication called levodopa, which increases the amount of dopamine in the brain. Physical therapy is also helpful to improve movement and balance. Your doctor may have you try adaptive technology, such as weighted utensils or gloves.
You may need medications to treat other symptoms, such as antidepressants for depression or stool softeners for constipation. Surgery, such as deep brain stimulation, can reduce the physical symptoms of Parkinsons disease in some people.

Are you on Oxaliplatin? That gave me extreme cold sensitivity as a side effect. It would affect my whole body on the day of chemo, but then just my hands and feet for a few days after. My daughter bought me a pair of light gloves with finger pads to let me text and scroll through my phone during those times. They helped a lot. You should report this to your oncologist and ask if there is anything you can do to reduce the side effects. But if you are on Oxaliplatin, make sure you report any signs of neuropathy in your fingers as soon as you start feeling them. My oncologist took me off that one drug when I started experiencing neuropathy . He said that if I stayed on it, the neuropathy would only get worse and be irreversible. Because we acted quickly, my neuropathy is mild and has slowly improved over the past 10 months. I’m sorry you’re going through this.

What Are Common Side Effects Of Chemo
Most people worry about whether theyll have side effects from chemo, and, if so, what theyll be like. Here are some of the more common side effects caused by chemotherapy:

Commonly, patients with psychogenic tremor often undergo a large number of diagnosis and therapeutic procedures before the final diagnosis is established. A review of medical history in these patients usually shows multiple functional somatic or psychosomatic illnesses. Once the diagnosis is made, most patients continue to have a fluctuating or constant course, followed by improving and progressive periods suggesting the prognosis is far from benign. The therapeutic success is also variable, but the treatment approach should include various combinations of psychotherapy as well as drugs, such as mild anxiolytics and antidepressants. While pharmacological treatment in organic tremor may reduce amplitude, but does not change the tremor frequency, the effect of treatment in psychogenic tremor usually varies from total suppression of tremor, especially when associated with the suggestion of a cure to no benefit. Interestingly, most of successfully treated patients were young.

What Are Common Side Effects Of Chemotherapy
The side effects you experience while on chemotherapy depend on which drug or combination of drugs are prescribed. Different drugs cause different side effects. And each person’s experience is different. People may not experience the same side effects even when taking the same drug. And you can have different side effects than you did in the past if you take the same drug again. That is why it is important to talk with your cancer care team regularly about the side effects you are worried about or are experiencing. Tell them about all the side effects you notice. It may be helpful to track side effects over time and share that information with your care providers. Common side effects of chemotherapy include:
Fatigue. Fatigue is feeling tired or exhausted even if you get enough sleep. It is the most common side effect of chemotherapy. Learn about how to cope with fatigue.
Hair loss. Some types of chemotherapy, but not all, cause hair loss. Hair on your body may come out a little at a time or in large clumps. What Causes Essential Tremor And Is There Anything That Triggers It
Experts dont know exactly why essential tremor happens or if there are triggers that cause them to happen. However, theres evidence that it happens because of the changes in certain parts of your brain.
And while experts dont know exactly why essential tremor happens, they do know that this condition can run in families. About half of all essential tremor cases are inherited. This condition appears to be an autosomal dominant inherited condition. That means if one parent passes a mutated gene that causes essential tremor to you, theres about a 50% chance that youll inherit the condition.
Parkinsons Tremors Vs Essential Tremors
Because they can be similar to Parkinsons tremors, essential tremors are often confused as symptoms of the disease. Just as with Parkinsons, essential tremor can cause uncontrollable rhythmic shaking in different parts of the body.
Up to 10 million people are affected by this common nervous system disorder. While genetics and environment likely play a role in essential tremor, the cause is unknown, according to the U.S. National Library of Medicine.
Essential tremors in the hands or arms can be distinct from Parkinsons because they typically happen when the hands are in use.
The essential tremor can get really bad when youre using your limb when drinking or eating soup, for example, says Gilbert. The Parkinsons tremor is usually not as disabling whereas the essential tremor can be life-altering.
The shaking from an essential tremor typically improves when using both hands to bring a cup to the mouth but the same action can amplify the tremor in Parkinsons, according to Gilbert.
Dr. Beck points out that an essential tremor may be faster than a Parkinsons tremor, which tends to be milder. A difference can often be seen in a persons handwriting. Those with essential tremor tend to have more unsteady and wavy writing, whereas Parkinsons patients are more apt to display micrographia, or abnormally small handwriting.
They have low amplitude movement so their writing gets smaller and smaller to a point where it can be barely legible, he says.

Chemotherapy Drug Interactions And Side Effects
When looking at how best to combine chemo drugs, doctors must look at interactions between chemo drugs and other medicines the person is taking, including over-the-counter medicines, vitamins, and supplements. These interactions may make side effects worse and affect how well chemo drugs work.
Its important that you tell your doctor about all medicines, including over-the counter medicines, vitamins, herbal or dietary supplements you are taking even if you only take them as needed.
For instance, platelets help blood clot and prevent bleeding. Many chemo drugs lower the number of platelets for a time. Taking aspirin or other related drugs can also weaken blood platelets. This isnt a problem for healthy people with normal platelet counts, but if a person has low platelet counts from chemo, this combination might put them at risk of a serious bleeding problem.
Your doctor can talk with you about the safety of using other medicines, vitamins, and supplements while you are being treated for cancer.
The Right Diagnosis Can Save Time
Because the symptoms of Parkinsons vary and often overlap other conditions, it is misdiagnosed up to 30% of the time, Dr. Fernandez says. Misdiagnosis is even more common in the early stages.
Patients who dont know where to turn may make appointments with a rheumatologist, or an orthopaedic or heart specialist, and undergo MRIs, EMGs and other expensive tests.
But only a neurologist can distinguish Parkinsons from essential tremor, drug-induced Parkinsons and Parkinsons plus syndromes, he says.
If patients come to us with typical signs of Parkinsons, we dont need to order expensive tests, he says.
Instead, neurologists base their diagnosis on a detailed patient exam and medical history, along with other information from the patient, family members or caregivers.
Thats all stirred into the pot, he says. Sometimes we can diagnose Parkinsons with one visit. Other times, several follow-up visits are necessary.

B Common Pitfalls And Side
For the tremor of true idiopathic Parkinsons disease, here are some rules for using the drugs above. In general, if a patient is over 65, you can start immediately with Sinemet . If under 65, you should start with a dopamine agonist. Though the dopamine agonists are overall less effective than Sinemet for the overall treatment of the disease, for the symptom of parkinsonian tremor, they are perhaps slightly more effective than Sinemet. Trihexiphenidyl, amantadine, and MAO-B inhibitors have some efficacy for parkinsonian tremor as well but to a lesser degree.
Vascular parkinsonism does not respond to anti-parkinsonian medications. Medication-induced parkinsonism responds very poorly to antiparkinsonian medications, and you quickly drive side effects.
By far the most effective drugs are propranolol and primidone. Choose the agent based on side effect profile and the patients other comorbidities. If the patient has lung disease , then avoid propranolol and choose primidone first. Likewise, if the patient has insulin-dependent diabetes, your first choice should be primidone. If the patient is elderly/demented or young/working/subject to drug screens, you may choose propranolol first. Primidone is a pro-drug for and metabolized to phenobarbital. Titration of propranolol can be limited by bradycardia or hypotension. Other agents can be added to these if this is insufficient.
