Factors Influencing Health Status And Contact With Health Servicesnote

  • Z codes represent reasons for encounters. A corresponding procedure code must accompany a Z code if a procedure is performed. Categories Z00-Z99 are provided for occasions when circumstances other than a disease, injury or external cause classifiable to categories A00-Y89 are recorded as ‘diagnoses’ or ‘problems’. This can arise in two main ways:
  • When a person who may or may not be sick encounters the health services for some specific purpose, such as to receive limited care or service for a current condition, to donate an organ or tissue, to receive prophylactic vaccination , or to discuss a problem which is in itself not a disease or injury.
  • When some circumstance or problem is present which influences the person’s health status but is not in itself a current illness or injury.

    • Drugs that have been approved by the FDA for the treatment of lung cancer

    Finding A Specific Code

Some articles contain a large number of codes. If you are looking for a specific code, use your browser’s Find function to quickly locate the code in the article. Sometimes, a large group can make scrolling thru a document unwieldy. You can collapse such groups by clicking on the group header to make navigation easier. However, please note that once a group is collapsed, the browser Find function will not find codes in that group.

Things To Remember When Coding Pci Cases

*The following information is intended for those using ICD-10

Coding prophylactic cranial irradiation cases can sometimes be challenging. PCI is often done following a course of radiation to the lung for patients with small-cell lung cancer who do not have evidence of brain metastasis. Here are a few things to remember when coding and documenting PCI cases.

  • You have to indicate to the patients payer that the treatment is prophylactic. This is typically done by using ICD-10 Z29.8 . It is preferably placed before the primary tumor diagnosis but some carriers prefer it after the primary tumor diagnosis.
  • Make sure the patients medical record indicates a response to the primary SCLC treatment and current performance status. Both of these items are often required to be at a certain threshold for PCI to be covered.
  • Payers often have very specific requirements set out for PCI. It is important to know the patients payers specific rules, especially when the payer does not require prior authorization for radiation therapy.
  • PCI claims require appeals more often than other radiation therapy claims. Therefore, it is important to make sure every I is dotted and every T is crossed in the patients medical record.
  • If you need to send in medical records for prior authorization or for claims payment make sure to include pathology information.

Adjuvant Chemotherapy for T3 NonâSmall Cell Lung Cancer with Additional ...
    • Cancer registrars should always code the histology strictly based on the terms used by thepathologist , not based onwhether a case is eligible for AJCC staging.
    • Cancer registrars should assign the histology code independent of and before assessingeligibility to stage the case.
    • Do not assess eligibility for AJCC staging until you have assigned the histology code.AJCC and NCI SEER ask cancer registrars to disregard web postings or any other instructionscontrary to the above-mentioned principles.

See the case review instructions below for specific instructions about reviewing cases already coded.

In the coming weeks, AJCC and NCI SEER will work to ensure CAnswer Forum answers direct registrarsto use the SEER Solid Tumor Manual rules. Ask A SEER Registrar is the website to ask questions andresearch clarifications regarding histology coding rules. AJCC and NCI SEER will continue ourimportant collaboration and involve other national standard setters and cancer registry stakeholders.Ultimately, we are all striving for the same goal: reliable, accurate cancer data.
